- Description
A Registered Nurse and Community Health Worker visit older adults (65+) in their homes, in the community and at the office in collaboration with other health care staff and community agencies
Services include:
- help with medication review
- help with forms
- accompaniment to medical appointments as appropriate
- make connections to supportive community and social programs
- chronic illness and disease management
- help accessing housing support services
