Primary Care Outreach to Seniors


Description
Health workers visit seniors in their homes to provide support and link them to appropriate community resources
  • supports high risk seniors living at home
  • uses a health links approach
Works in collaboration with seniors' primary care providers and other service providers in the community to help seniors achieve their goals, to provide care and appropriate supports that empower vulnerable seniors to live at home
Eligibility
Seniors 65 years and older who may be isolated and at risk of hospitalization
Application Process
Call central intake to inquire
* a completed Primary Care Outreach Referral Form must be faxed to 613-739-8196 or submitted online

Completed Primary Care Outreach Referral Form * form must include all information including the name, organization and contact details (phone , fax & email) of the referral source
