Description
Inpatient and outpatient assessment and rehabilitation care
  • improves the patient's daily functional abilities through assessment and treatment of physical, cognitive, medical and social issues

Specific programs include:
  • stroke rehabilitation
  • Care of the Elderly
  • physical medicine and rehabilitation clinic
  • ambulatory stroke rehabilitation service
  • geriatric rehabilitation (see separate record for details)
  • The John and Jennifer Ruddy Geriatric Day Hospital (see separate record for details)
Eligibility
Patients who have had a recent decline in their ability to manage everyday activities

Residency Requirements: Champlain District LHIN boundaries
Application Process
Physician referral required
* referrals from sources outside the usual catchment area will be considered
* Referrals should be completed using the provincial RM & R form for inpatient programs
