Ottawa Paramedic Service - Community Paramedicine Program
- Description
Paramedics operating in an expanded role to provide in-home assessments and care to members of the community with pre-scheduled and urgent visits with the goal of stabilizing acute and chronic medical conditions. Community Paramedics provide health assessments, including:
- blood pressure
- heart rate
- respiratory rate and assessment
- oxygen saturation
- blood glucose
- ECG monitoring
- fall risk assessments
- home safety scan
- medication reviews
- treatment planning and associated tests
- care coordination and integration with health teams
- communication with Primary Care Physician or Nurse Practitioner
- coordination with community programs and referrals to support services
