Outreach services, assesses the needs of homeless people in Ottawa

• Transportation: Emergency based transportation service for homeless and shelter-staying clients such as
  • transport from street to shelter
  • shelter to more appropriate shelter
  • to and from hospitals where an ambulance is not needed and a nurse or social worker can confirm the client has been seen and discharged by a doctor
    
• Supplies (for clients who live on the street and choose not to come in a shelter only): Life-sustaining necessities such as a sleeping bag, boots, jackets, socks, gloves, water, etc. * does not provide food
  
• Referrals/Assessment : Can meet the client at their location and provide an assessment for advocacy or referral to the social services network of Ottawa

• Homeless individuals 16 years and older in an emergency situation
• Transportation: There must be a demonstrated need for the service such as the client cannot bus, is too intoxicated to travel safely or has no other means to get there
• Not for transportation to and from private residences and the client must be mobile enough to get in and out of the vehicle on their own
