Description

Transportation services for older adults and people with disabilities living in the East of Ontario Health -East Region

Provided by a collaborative of 25 Community Support Service (CSS) organizations, divided into four catchment areas
Help with transportation to, from:

  • medical appointments
  • discharge from hospital
  • day programs
  • occupational therapy (OT) assessment
  • dialysis treatment
  • oncology treatment
  • some agencies can provide drives for daily living activities such as work, grocery shopping, banking, Diner's club, pharmacy, etc.

Centers need between one to seven days notice for transportation

Eligibility
Individuals 65 years or older and adults 18 years and older with disabilities

Some exceptions can be made in rural regions
