Transportation services for older adults and people with disabilities living in the East of Ontario Health -East Region
Provided by a collaborative of 25 Community Support Service (CSS) organizations, divided into four catchment areas
Help with transportation to, from:- medical appointments
- discharge from hospital
- day programs
- occupational therapy (OT) assessment
- dialysis treatment
- oncology treatment
- some agencies can provide drives for daily living activities such as work, grocery shopping, banking, Diner's club, pharmacy, etc.
Centers need between one to seven days notice for transportation
- Eligibility
- Individuals 65 years or older and adults 18 years and older with disabilities
