Transportation services for older adults and people with disabilities living in the East Ontario Health Region
Provided by a collaborative of 25 Ontario Health atHome organizations, divided into four catchment areas
Help with transportation to, from:- medical appointments
- discharge from hospital
- day programs
- occupational therapy (OT) assessment
- dialysis treatments
- oncology treatments
- some agencies assist with grocery shopping and other activities of daily living
Centers need between one to seven days notice for transportation
- Eligibility
- Individuals 65 years or older and adults 18 years and older with disabilities
Some exceptions can be made in rural regions - Application Process
