- Description
- Program is designed to help parents of children with disabilities or other identified needs ages 0-6 years to access programs and services that they need. Children ages 7 to 18 may also qualify for services if there are special circumstances related to an identified needs in the areas of health, education and social.
Program goals include:- Provide a link between the home and the physician
- Get assistance and/or support from the most appropriate caregiver, at the right time in the most supportive way possible for the families involved
- Provide support and advocacy services to families of children with special needs by assessing the situation, establishing a plan with the parent(s), making referrals to appropriate services, home visits and other case management duties as required
- Support families with individual grant applications and inform families of the help available to them and their child.
