Access to health care support at home, at school or in the community. Coordinates admissions into long-term care homes (including short-stay and respite care beds) and into complex continuing care (chronic care)

Coordinates in-home services:

Coordinates in-school services:

  • palliative care
  • respite care

Fraudulent Automated Calls to Patients: some home care patients have received fraudulent automated telephone calls informing them they would be discharged from home and community care services, but giving them the choice to purchase additional services.
