Community Mental Health Program - Flexible Assertive Community Treatment Team
- Description
Teams are comprised of interdisciplinary mental health professionals who work with patients to develop recovery goals and help patients with housing, school or jobs, life skills, relationships and integration into their communities
- also provides treatment, medication monitoring and ongoing assessment of mental illness concerns
- works closely with other care providers, including family physicians, drop-ins, staff from supportive housing and inpatient teams, to provide the best possible care to their patients
- emergency after-hours on-call system for registered clients of the program
- Eligibility
- Individuals with complex, long-term and serious psychiatric illness involving multiple hospitalizations of a minimum of 50 days in past year or 150 days over 3 years * a client assessment will be done to determine eligibility

- An individual, the family or the current service provider (including health care providers) can make referrals by completing the FACT Central Intake Referral Form and faxing it to FACT Central Intake at 613-739-8400
