Clinic locations include Hospital for Sick Children (Hematology / Oncology Clinic) and Toronto General Hospital (Division of Hematology). Treatment is also available through Hamilton Health Sciences / McMaster University Medical Centre. - Eligibility

- Toronto General Hospital, Division of Hematology: adults
- McMaster University (Division of Hematology and Thromboembolism): clinical care for children and adults
