Description
Outpatient treatment, support and rehabilitation for people with severe and persistent mental illness. Services include:
  • medication monitoring and supervision
  • crisis assessment and intervention
  • symptom assessment, management and education
  • supportive counselling
  • collaboration with families and agencies
  • life skills teaching
  • housing assistance
  • recreational support
  • vocational support
  • continuity of care
  • advocacy

Multidisciplinary treatment includes psychiatrist, nurses, social workers, occupational therapists, recreation therapists, addictions workers.

Referrals can be arranged directly or through Here 24/7 service.
Eligibility
Clients with severe and persistent mental illness. Call or visit website for specific program admission criteria.
Ages: 16 years - 64 years
