- medication monitoring and supervision
- crisis assessment and intervention
- symptom assessment, management and education
- supportive counselling
- collaboration with families and agencies
- life skills teaching
- housing assistance
- recreational support
- vocational support
- continuity of care
- advocacy
Multidisciplinary treatment includes psychiatrist, nurses, social workers, occupational therapists, recreation therapists, addictions workers.
Referrals can be arranged directly or through Here 24/7 service. - Eligibility
