Specialized Geriatric Mental Health Services
- Description
- Assessment and treatment for people over 65 years of age who have a deteriorating complex health condition that includes mental, physical and/or behavioural difficulties. Services are primarily provided through home visits and visits to long-term care facilities.
- Eligibility
- Individuals with mental health concerns who are living independently, in nursing homes or residences * their families
Ages: 65 years and up - Application Process
- Receives referrals via Specialized Geriatric Services Centralized Intake located at St. Joseph's Health Centre in Guelph. These referrals will be reviewed by the Geriatric Mental Health team located at the Langs Hub.
