- Interim mental health and addiction support for people transitioning into the community after inpatient psychiatric care or following a recent mental health crisis.
Services may include:- phone support
- individual multidisciplinary team assessment
- consultations and contacts
- group education and individual support sessions
- support for medication management

Ages 18 years - 65 years - Application Process
- Homewood in-patient referrals to Transitional Care are made through the in-patient clinical teams of Homewood Regional Services.
Community Access via 24-Hour Crisis Supports: Emergency departments at Guelph and Wellington local hospitals, and contact with the mental health crisis nurse.
Persons in crisis may also contact HERE 24/7 at 1-844-437-3247.
