Processes registrations for consent to donate human organs and tissues. Donors will receive a confirmation letter when they:
- complete a new registration
- update their registration information
- withdraw their consent
- Eligibility
- Donors must be 16 years of age or older and have a valid Ontario Health Card.
- Application Process
- Register, check, or update consent online. Registration is also available in person at any
ServiceOntario centre or by completing the Gift of Life consent form and mailing it to the address on the form. - Documents Required
