Non-emergency, community-based service supports vulnerable patients living independently in the community.
Services include:- In-home wellness checks
- Remote patient monitoring - blood pressure, heart rate, oxygen saturation and blood glucose can be collected at each visit
- Connections to local services

Patients who require additional assistance while living in their home, including high-risk clients living with complex medical issues such as diabetes, Chronic Obstructive Pulmonary Disease (COPD) and Congestive Heart Failure (CHF)
The person being referred to this service lives in Peterborough City or County, the patient is at risk of entering long-term care or is on a long-term care waitlist and/or the person has ongoing chronic conditions that are not managed well and are at risk for hospitalization. - Application Process
- Both Self-referral and Health Care or Social Services Professional Referral Possible. Referral to paramedic services through a caregiver, hospital, physician, or with self signup through the website.
