Description

Medical Condition Management

  • Responding to changing conditions and connecting patients to appropriate health care providers or social services to prevent crises.

Clinical Interventions

  • Performing diagnostics (specimen collections, 12/15 lead ECGs, vital signs monitoring, POC testing).
  • Providing minor wound care, observational assessments, and focused evaluations.
  • Conducting other controlled medical procedures and treatments at home. If request is urgent, submit referral and call 613-771-9366, ext. 300 to speak with an on-duty Community Paramedic

Remote Care Monitoring

  • Offering remote monitoring for continuous care and timely interventions.

In-Home Medical Procedures

  • Vaccinations. Administering vaccinations for homebound patients.

Eligibility
A patient is eligible for enrollment in the Community Paramedic Program if they meet the following criteria: The patient has a chronic medical condition that is not well-managed, such as:
  • Acute Exacerbation of Chronic Obstructive Pulmonary Disease (AE COPD)
  • Congestive Heart Failure (CHF)
  • Non-Insulin Dependent Diabetes Mellitus (NIDDM)
  • and the patient is assessed to be at high risk of requiring emergency medical services due to their medical condition.
