Services include:- In-home wellness checks
- Remote patient monitoring - blood pressure, heart rate, oxygen saturation and blood glucose can be collected at each visit
- Connections to local services

- Client is an older adult who is on a LTC waitlist, is eligible or soon to be eligible for a LTC waitlist and is a Northumberland resident.

- Referral to paramedic services through a caregiver, hospital, physician, or with self sign up through the website.
