- offers consultation, coaching, mentoring, and education to primary care providers in the Champlain Region
The Regional Palliative Consultation Team is a partnership between Bruyère and Champlain Ontario Health at Home, supporting family physicians and primary health care teams caring for palliative patients in the community. This includes care offered at home, retirement homes, long-term care facilities and community hospitals.

- Primary care providers, community hospitals with no palliative care services, long-term care homes, retirement homes, nurses and other health professionals providing end-of-life care in the community, and care coordinators in the Champlain region, who need help to support patients with a life limiting disease
